I mean if MC can cheat on his girl friend regardless of who she is, there can be a plot about a conflict with Tommy. Maybe Tommy will go after the girls that MC cheats on for example if there is a cheating pathway.

After all, Heather left Tommy because of the cheating. I doubt a cheater MC will have a good time with her unless it's a serious committment. She also sticks around a lot and requires effort and care.

In my opinion it can be a serious plot worth the investment but it seems like that's going to Lily instead...

In almost every relationship in this game that we know of there is at least 1 cheater. You're also forgetting one of the biggest ones that MC can join: Jade and Stephen.

Also how is John Boy cheating? He and Elena only have sex with the other present, right?

Nah, unfortunately this is just for funny dialogue, unless DPC "forgot" to program in the variables, which I currently doubt even with the kissing Sally and Bianca route "miscoding".
